• Download International application form
  This document contains the forms and guidance notes that will make up your application. Please ensure that you also include the course information form, below, with your application form.
  
  
• Download Course information form
  This form is for information about your course. In order to use it, you will need to download it, and email it to your University or Institution. They can then fill it in electronically, print it out and stamp it, then send it back to you. You will then need to include it with the rest of your application.
  
  
• Equality and Diversity monitoring form
  We hope you will help us by completing this form but please be aware the form is entirely voluntary and, if you choose not to complete it, this will not affect your application for registration.Please do not re-scale this form when printing.
  
  
• Download Standards of Conduct, Performance and Ethics
  These are standards that you must agree to keep to in order to be registered.
  
  
• Download Standards of Proficiency
  These are the standards that the assessors will use to assess your application.
  
  
• Download Standards of Continuing professional development
  These are the professional standards which you must meet in order to stay registered.
