Mrs Kielye L Mitchell faces allegations that her fitness to practise is impaired by reason of her misconduct and/ or lack of competence. The allegations include she did not identify and recognize the severity and deteriorating health condition of a patient, and did not document the correct symptoms, did not transport a patient to hospital under blue light conditions and/or pre alert the hospital prior to her arrival.

Mr Peter J Fletcher faces allegations of misconduct and/or lack of competence whilst registered as a Physiotherapist. Allegations include he did not maintain adequate patient’s records, sent a password-protected document to patient but did not provide the password and indicated that hospital prevented him from disclosing patient notes when that was not the case.
