Mr Dennis Gilbert Howard's fitness to practise is alleged to be impaired by reason of misconduct and lack of competence. While registered as a social worker, during May 2017, he did not attend a court hearing for two children or complete preparation for final evidence for court proceedings within the time frame required. He did complete a specific assessment for the children and failed to visit them at home when required during June 2017. He also failed to complete a risk assessment for another child in June 2017 and did not record a statutory children in care visit.

Mrs Angela Bell's fitness to practise is alleged to be impaired by reason of misconduct and lack of competence. Whilst employed as a social worker she did not follow the legislative process under the Mental Capacity Act in relation to a service user. She also falsified the service user's records in an assessment during September 2016.

Miss Dorothy Ribombo's fitness to practise is alleged to be impaired by reason of misconduct. Whilst registered as a social worker, in or around April and May 2017 she failed to visit a child within statutory timescales. During April and May 2017 she also failed to follow reasonable management and supervision instructions for a number of children.

Mrs Winifred Janet Little's fitness to practise is alleged to be impaired by reason of misconduct and lack of competence. Whilst registered as a social worker, during October 2015 and June 2016, she failed to complete and / or record statutory visits for a number of children. She also prematurely completed an assessment before a child was born.

Mrs Jacqueline Ann-Marie Wedderburn-Crooks's fitness to practise is alleged to be impaired by reason of misconduct. Whilst registered as a social worker, during August 2016 and October 2016, she telephoned a family member of a service user and spoke unprofessionally about the local children's services. She remarked that they were "ruthless bastards" and alleged that the local children's services had discriminated against the service user’s mother because of her mental health. She supplied confidential information to the service user's family which resulted in an emergency protection order in relation to the service user. She breached professional boundaries by using her personal telephone to call the family despite having her work mobile removed from her during October 2016. On an unknown date, she also spoke in an unprofessional manner to another service user by stating she was “street” or words to that effect. During the period of her employment with a local borough council she progressed only 4 of 28 cases for which she was responsible.

Mrs Michele Gerard's fitness to practise is alleged to be impaired by reason of a conviction. Whilst registered as a social worker, during January 2019 she was convicted of causing death by careless or inconsiderate driving.

Mrs Alice Treharne’s fitness to practise is alleged to be impaired by reason of misconduct and lack of competence. Whilst registered as a speech and language therapist she did not undertake or record appropriate reviews and / or assessments for 10 patients between April 2010 and July 2016.

Mr Jeffrey S Horn’s fitness to practise is alleged to be impaired by reason of convictions. Whilst registered as a radiographer, during October 2018 and November 2018 he was convicted of rape and four counts of cruelty to a person under 16 years.
