Requesting a copy of your application or registration details
The main route for requesting copies of personal information is under the Data Protection Act 1998 - where a Subject Access Request is the main mechanism.
To cover the costs of locating your details and making a copy, we charge you £10 for each set of information that you request. By law, we must provide this information to you in no more than 40 days.
If you want information about your registration or application, this is called making a ‘Subject Access Request’. To do this, you should write to us, stating clearly in your letter that you wish to make a subject access request. You need to provide your full name, correspondence address, date of birth, registration number (or application number if appropriate) and you must sign the letter. All details will be checked against the information that we already hold. If your letter does not contain all of this information, or if you contact us by phone or email, we will not be able to process your request.
If we have any queries regarding a Subject Access Request, we will write back to you to ask for more information. This is to make sure that your registration or application information is not sent to anyone else.
Please note that references relating to applications (the health and character references) will remain confidential and the sole property of the HCPC.
